Abstract
A 56-year-old male developed left anterior ischemic optic neuropathy (AION) after surgery to remove a left sphenoidal ridge meningioma fed by the left ophthalmic artery and left middle meningeal artery. He noticed loss of vision to light perception only in the upper nasal quadrant of the left eye immediately after the operation. His left fundus showed pale disc edema and 4 emboli in the retinal arteries, but no retinal edema. By 1 month after surgery, visual acuity had recovered to 20/20 and the visual field had a defect only in the lower nasal quadrant, but the optic disc became atrophic. The cause of Alan in this case was speculated to be emboli or insufficient vascular perfusion or a combination of emboli and insufficient perfusion. AION might have a good prognosis in cases of incomplete occlusion with some emboli and temporary, mild insufficiency of vascular perfusion.
