抄録

A male patient in his 60s with a history of distal gastrectomy (Billroth Ⅱ) for duodenal ulcer 40 years ago. Fecal occult blood was positive, and endoscopy revealed a type 3 tumor at the anastomotic site. He was referred to our hospital and diagnosed with remnant gastric cancer and multiple liver metastases (cT4aN1M1, Stage ⅣB). He received 18 courses of SOX + nivolumab chemotherapy. Liver metastases disappeared, but PET-CT showed residual uptake at the primary site. Laparoscopic total remnant gastrectomy with D2 lymph node dissection was performed as a conversion surgery. Pathology showed no residual cancer. Chemotherapy with nivolumab was effective in enabling conversion surgery.
