A 71-year-old man who had undergone PCI several times previously was admitted to our hospital to treat a distal lesion of the RCA. At first we crossed a PT Graphix® into the culprit lesion smoothly. Next we tried to cross an IVUS catheter, but could only pass it into the middle of the RCA. It became entrapped and subsequently detached when we tried to pull it out from the RCA. In spite of using a goose neck snare, we couldn't remove the ruptured catheter. An emergent operation revealed that the IVUS catheter was twined around the struts of the stent which had been implanted in the middle of the RCA two years before.
