We report a case of malignant acanthosis nigricans in a 63-year-old man. He was hospitalized for the operation of the advanced gastric cancer. Hyperpigmentation and rough skin were found on his face, neck and axillas, and wartlike change of the skin were found on his palms and soles at hospitalization. Histological findings of the hyperpigmented lesion revealed papillomatosis, acanthosis of epidermis and hyperpigmentation on the basal layer. After the operation of gastric bypass, the eruptions were not changed. Twenty four cases (71.4%) in 35 malignant acanthosis nigricans were associated with gastric cancer from 1989 to 1998 in Japan. The tendency in association of malignant acanthosis nigricans with gastric cancer is decreasing in Japan.
