A 34-year-old male presented with a 5-year history of multiple subcutaneous nodules on his extremities and trunk. He was a professional cycle racer and had experienced frequent falls during practice. Histology of the biopsied nodule revealed an encapsulated and honeycombed structure. The capsule was composed of fibrous connective tissue and degenerating fat tissue. We diagnosed him as having multiple lesions of nodular-cystic fat necrosis in the middle stage. In Japan, there have been no previous reports of multiple fat necrosis in the extremities and trunk. As the patient had a history of repeated trauma from cycle racing, we propose that the inciting factor was trauma.
