We would like to present a case of pulmonary aspergillosis with simultaneous lung and liver cancer. A 73-year-old man was diagnosed with pulmonary aspergillosis from a lung mass. The patient partially responded to anti-fungal drug treatment. Unfortunately, chest CT performed 7 months later revealed progression to the lung cavity at the right upper lobe. A bronchoscopy, performed for tissue diagnosis, established a diagnosis of adenocarcinoma. A subsequent blood test revealed a high level of carcinoembryonic antigen (228.5 ng/ml). We investigated the primary source of the carcinoma by performing CT and PET on the whole abdomen. Based on our findings, we made a definite diagnosis of hepatocellular carcinoma and simultaneous tumor with adenocarcinoma of the right lung that correlated with a staging of T4N2M0 (Stage IIIB). However, these cancers were comorbid with pulmonary aspergillosis.
