Thirty-one patients with loco-regional recurrence of rectal cancer were treated with high dose (≫50Gy) external radiotherapy during the period from 1994 to 2000. We analyzed the treatment outcomes, prognostic factors for local control, relief of symptoms and complications. Multivariate analysis was applied to identify possible prognostic factors for local control. Pattern of pelvic local recurrence was the only variable that significantly influenced local control. Six patients (19%) developed grade 3-4 late morbidity. In conclusion, high dose external radiotherapy (≫50Gy) was effective for the localized pattern of recurrence. The optimal high dose radiotherapy should be explored for patients with the localized recurrence.
