Purpose : To report a case implanted with intraocular lens (IOL) by transscleral suture who manifested cystoid macular edema (CME) during topical pranoprofen that resolved after topical diclofenac. Case : A 63-year-old male had received cataract surgery with IOL implantation 14 years before. He had received transscleral IOL suture for displacement 8 months before. He had reportedly developed CME thereafter and had been receiving instillation of diclofenac. Findings : The right eye showed CME with visual acuity of 0.4 CME resolved 4 weeks later. Diclofenac was switched to pranoprofen. CME recurred another 5 months later. Pranoprofen was switched to diclofenac. CME resolved 10 weeks later. Conclusion : This case illustrates that topical diclofenac may be effective for CME in Pseudophakie eye that is refractory to topical pranoprofen.
