Purpose: To report a case in which scleral buckling was performed with the use of an optical fiber-free intravitreal surgery system (OFFISS) and to discuss the usefulness of this system. Methods: A 50-year-old female underwent scierai buckling with the use of an OFFISS, to treat rhegmatogenous retinal detachment. The whole procedure, including cryopexy and fundus examination, was video-recorded digitally via a camera attached to the surgical microscope. Results: There were no complications during surgery. The retina re-attached and visual acuity was preserved. The video image was very useful because it served as the surgical record. Conclusions: The OFFISS was useful as a digital record of surgery in this case and it could also be useful for education, because it recorded the fundus examination as well as the whole surgical procedure.
